Please sign up with us by providing your Extended Health information to our Customer Care Coordinator prior to your massage.
Once you have signed up and signed the consent forms we then keep the information and bill for you each visit to the extent that your company allows.
It is recommended that you check your Extended Health Plan for the following:
Does your plan require a note from your doctor?
How much coverage does each family member have each year?
When does your year begin and end?
What percentage does your plan cover per treatment?
Does your plan allow payment to come directly to us or does it pay only to the plan member?
Loyalty points are earned each time you have a massage at Medi-Massage! We record your points automatically.
